Forms

Any registered BCGHA Player who wishes to skate for another association must have a Permission to Skate form to present before she is allowed on the ice.  Permission to Skate forms can be obtained by submitting a request using the form below. As instructed by the OWHA, all Permission to Skate forms are issued as originals only and will not be faxed or emailed.

Permission to Skate forms will be issued periodically, excluding the period from January 1 until after the Ontario Provincial Championship weekend.  This is a volunteer organization, and any requests submitted will be available for pick up within one week or can be mailed to you. As no Permission to Skate forms are valid between the end of regular season and the end of the Provincial Championship, any requests submitted during this time will be available for pickup the week following Provincials.

Requests for Permission to Skate are controlled by Google forms. The confirmation screen which is displayed after submitting the form is confirmation that the request has been received. You will not receive a confirmation email. Any email or voicemail to the Secretary to request confirmation will not be acknowledged or responded to. Trust the process.

Player Release forms are no longer requested by players. A player release is requested through the OWHA registration system by the association wishing to acquire the player. Releases are issued directly to the association electronically.  Releases will only be granted to players for whom all team and Canadettes dues have been paid in full.
